Although diabetes care has been widely established as a single entity, data in older, frail individuals with numerous comorbidities and have limited polymedication. This group accounts for sizable number of nursing home residents (NHs). We came together experts multidisciplinary (endocrinologists, geriatricians, Nurses staff, general practitioners diabetologists) real-world practice this field, which is becoming more important globally, to compile practical, straightforward guidance on the managing senior, fragile patients diabetes. An Individual Care Plan (ICP), presented layman's terms, cornerstone patient's treatment. due demands placed personnel NH (medical coordinator, manager, nurses, caregiver working at front lines, other worker under training). The actual purpose document, released when patient admitted, made prescriptions ensure that given followed after admission. It details need proper treatment, regular monitoring, dates times essential examinations testing. entails keeping tabs HbA1c, well their blood urine glucose levels any complications may arise from condition (hypoglycemia, cardiovascular disease, foot disorders, ocular problems, malnutrition, kidney failure peripheral neuropathy). Consequentially, staff education unique challenges an elderly person diabetes, emergency procedures, maintaining up-to-date ICP use by medical professionals essential.
